Vaping and lung capacity: what the evidence says

We analyse the impact of vaping on the lungs compared with combustible tobacco, separating harm reduction, lung function, respiratory symptoms and the limits of current evidence.

Vaping and lung capacity compared with combustible tobacco

1. Science vs myth

The right question is not whether vaping is “good for the lungs”. The right question is: what happens when an adult smoker stops inhaling tobacco smoke and replaces it with vaping.

Combustible tobacco produces smoke through combustion. That smoke contains tar, carbon monoxide and thousands of toxic substances. An e-cigarette does not burn tobacco: it heats an e-liquid and produces aerosol. This does not make vaping risk-free, but it does substantially change the exposure profile.

Responsible message

For a non-smoker, the best option for the lungs is not to smoke and not to vape. For an adult smoker who cannot stop smoking, completely replacing cigarettes with vaping can significantly reduce exposure to combustion smoke.

2. Tobacco smoke vs vape aerosol

The key difference is combustion. Burning tobacco and heating an e-liquid are not equivalent processes.

Combustible tobacco

Combustion generates tar, carbon monoxide and a complex mixture of toxic substances associated with lung and cardiovascular harm.

Nicotine vaping

Aerosol can contain irritants and nicotine, but it avoids direct tobacco combustion and does not contain tar or carbon monoxide like cigarette smoke.

Not “clean air”

Lower exposure does not mean zero risk. The recommendation for non-smokers remains: do not start vaping.

3. What we know about vaping and lung function

The available evidence points to a clear difference between smoking and vaping in toxicant exposure. There are also follow-up studies in e-cigarette users, although we still do not have decades of data like we do for tobacco.

3.5-year prospective study

In a small cohort of daily e-cigarette users who had never smoked, researchers detected no significant changes in lung function, respiratory symptoms, exhaled nitric oxide or pathological findings on high-resolution CT during follow-up.

Public health reviews

Public health reviews maintain that vaping poses only a small fraction of the risks of smoking, especially in the short and medium term, while stressing that it is not risk-free.

Respiratory symptoms

Some users may experience coughing, irritation or dryness, especially at the beginning, with high nicotine strength, high PG, excessive power or poor inhalation technique.

4. What cannot yet be claimed

The evidence allows us to compare relative risk with smoking fairly clearly, but it does not allow us to say vaping is completely safe for the lungs.

No decades of follow-up

Tobacco has been studied for many decades; modern vaping has a shorter history and still needs long-term monitoring.

Not all products are equal

E-liquid quality, device type, power, coil condition and correct use all influence the user’s real exposure.

Not for non-smokers

The risk-benefit balance changes completely if the person has never smoked. For non-smokers, adding vaping provides no health advantage.

5. Practical tips for adult smokers

Replace tobacco completely

Reducing cigarettes is progress, but the biggest change comes when tobacco smoke is no longer inhaled at all.

Use suitable power

Do not push coils outside their recommended range. A burnt coil causes bad taste, irritation and a worse experience.

Choose nicotine carefully

If nicotine is too low, you may return to cigarettes. If it is too high or used in a powerful device, you may feel throat irritation, coughing or dizziness.

Avoid vaping near sensitive people

Although it is not comparable to second-hand tobacco smoke, as a precaution avoid vaping near babies, children, pregnant people or anyone with respiratory problems.

Frequently asked questions about vaping and lung capacity

Does vaping improve lung capacity?

It should not be promised as a direct improvement. What may happen in an adult smoker is that, by stopping tobacco smoke inhalation, exposure to combustion toxicants is reduced and smoking-related symptoms may improve.

Is vaping less harmful to the lungs than smoking?

Available evidence indicates that vaping exposes users to fewer toxicants than combustible tobacco. Still, vaping is not risk-free and the best option for someone who does not smoke is not to start.

Why do I cough when I start vaping?

It may be due to poor inhalation technique, too much nicotine, high PG, excessive power or a worn coil. If coughing persists, check your device and speak to a healthcare professional.

Is second-hand vapour the same as tobacco smoke?

It is not equivalent to second-hand tobacco smoke. Still, out of respect and precaution, it is best not to vape indoors around babies, children, pregnant people or anyone with respiratory problems.
